Nourisher · Nutrition Agent

Keep kidney nutrition visits on the calendar

The Nourisher schedules visits with the practice's dietitian, sends approved education, and routes every diet question to the care team.

Beta

The Nourisher introduces itself as an AI assistant for your practice on every call and message, and hands off to your staff whenever someone asks.

The workflow

The Nourisher is in beta, and we configure it with each practice during onboarding. It coordinates nutrition care and gives no advice.

  1. 01

    Start from a care-plan trigger

A provider order or a new CKD stage on the chart marks a patient as due for a nutrition visit, under rules the practice sets.

  2. 02

    Offer the visit

The agent offers the patient a visit with the practice's own dietitian, inside the scheduling rules the office approves.

  3. 03

Route every question to the care team

Any question about diet, fluids, potassium, phosphorus, or medication goes to the care team with the patient's words attached.

  4. 04

    Prepare for the visit

Before the appointment, the agent sends reminders, education the dietitian has approved, and a food-log request if the practice uses one.

What the agent handles

Medicare Part B covers medical nutrition therapy for eligible patients with chronic kidney disease who are not on dialysis, yet the referral to a dietitian is easy to lose between a provider's order and a scheduled visit. For [nephrology practices](/specialties/nephrology/), the agent handles that scheduling and leaves the nutrition care to the dietitian.

  • Nutrition visits scheduled

Patients offered a visit with the practice's dietitian after a care-plan trigger.

  • Approved education sent

Only materials the practice's dietitian has reviewed and approved.

  • Visit reminders

Reminders on the practice's schedule, so the visit is kept.

  • Pre-visit food logs

Requested before the visit, for practices that use them.

  • Questions to the care team

Every diet, fluid, or medication question passed to a person.

  • Follow-up visits kept on cadence

Return visits offered when the dietitian or provider asks for one.

Coordination, never advice

The agent never gives diet or clinical advice of its own. It does not answer what a patient can eat or drink, how much potassium or phosphorus is safe, or anything about medication. Those questions go to the care team every time.

In beta, we set up the care-plan triggers and approved materials with each practice during onboarding.

  • No diet or clinical advice

It does not answer questions about food, fluids, labs, or medication.

  • Every question routed

A nutrition question becomes a care-team task with the patient's words attached.

  • Only approved materials

The agent sends education only after the practice's dietitian approves it.

  • The dietitian leads the care

Assessment, the nutrition plan, and any change to it belong to the practice's dietitian and providers.

What the practice can measure

Every nutrition referral leaves a record the team can review:

  • Patients due for a visit

    Added after a care-plan trigger.

  • Nutrition visits scheduled

    With the practice's dietitian.

  • Approved education sent

    By material and patient.

  • Questions routed to the care team

Every diet, fluid, or medication question.

  • Food logs returned

    Where the practice uses them.

These are recorded workflow outcomes, not promised clinical results.
