How the record works
Every nutrition referral moves through four recorded stages.
- 01
Care-plan trigger
The order or stage change that marked the patient as due.
- 02
Visit offered and scheduled
The offer made, and the visit scheduled with the practice's dietitian or the reason it was not.
- 03
Questions to people
Each diet, fluid, or medication question sent to the care team, with its status tracked.
- 04
Visit preparation
Reminders, approved education, and food-log requests sent before the appointment.
What the practice can review
The views the team works from:
-
Patients due
Who is due for a nutrition visit, and why.
-
Scheduled visits
Nutrition visits on the dietitian's calendar.
-
Education log
Which approved materials went to which patients.
-
Care-team queue
Patient questions waiting on a person.
-
Food-log status
Requested and returned, where the practice uses them.
-
Follow-up cadence
Return visits requested by the dietitian or provider.
How the numbers stay honest
The reporting describes scheduling and coordination. It does not report on a patient's diet, labs, or health, and it does not credit the agent with clinical results that belong to the dietitian and providers.
The Nourisher is in beta. We agree the reporting fields with each practice during onboarding.
-
No clinical metrics
Lab values, weights, and diet adherence stay out of this record.
-
Routing is reported steady
Every question going to the care team is the design, so that number never wears an up-is-good arrow.
-
Approved materials only
Education is counted only when the practice's dietitian approved it.
-
No PII in marketing analytics
No patient data, health details, phone numbers, insurance IDs, or free text, ever.
